Preparing for your initial evaluation

The following documents should be located to properly study, analyze, and prepare a personal plan for you. This material will be treated confidentially and returned when the planning process is complete, or earlier, if requested. Please bring as many of the following documents as possible to our first meeting.

Personal Files

  • Latest income tax returns
  • Mortgage Loan documents
  • Wills (if you have one, if not please let us know)
  • Trust agreements (usually drafted with wills)

Employer

  • Payroll or other income statements
  • Employee benefits booklet
  • Retirement savings plans
  • Pension plans

Bank or Credit Union

  • Checking account statements
  • Savings/CDs/Money Market account statements

Brokerage, Mutual Funds, and Investment Accounts

  • Latest monthly statements

Insurance Company                             

  • Latest quarterly and annual life insurance/annuity account statements
  • All individually owned Life Insurance policies or contracts
  • Health insurance/hospital and major medical policy information
  • Disability income insurance policy information
  • Property and casualty policy information
  • Long-Term Care policy information

Business                                              

  • Buy-Sell Agreements
  • Deferred Compensation Agreements
  • Stock/Option/Bonus Plans
